Visa Application Form: DS-160 form duly filled and signed

Interview Appointment letter : You need to be present at the US Embassy or Consulate where the Appointment for the Visa Interview is scheduled.

 Two recent passport size photographs with matt or semi matt finish,

       Passport with validity of  minimum 6 months and 2 blank pages for visa

      Two recent passport-sized photographs (45 mm x 35 mm) covering 80% face.

       Covering letter on letter pad if self-employed or on plain paper if employed

      Company ITR Last 3 years

     Family Details

      Fixed Assets (Fixed deposits, LIC, Property Papers)

       Education Proof—Last education place date  year

      Bank Statement for last six months with bank seal and sign on every page (Page size A4)

      Salary slips for last 3 months

     Proof of occupation – Self-employed:-company registration certificate, proof of proprietorship/ partnership deed, company profile, articles of memorandum                                                                                            

    Employed – leave letter, copy of employee’s ID Card, NOC from employer and copy of ID Card with signature of authorized signatory.

     Proof of accommodation:- hotel confirmation
